Simmons was greatly limited by an array of leg injuries in 2023, but he regained his health in 2024. His sack total of 5.0 was a disappointment on the surface, but Simmons played a career-high 342 snaps in run defense. That allowed him to log a career-high 76 tackles. That shift in role was likely due to game script rather than a preferred scheme by the Titans, so a higher sack total and lower tackle total is likely in the cards for 2025. Simmons posted seven tackles (four solo), including 1.0 sacks, one forced fumble and a one-yard touchdown catch during the Titans' 37-24 loss to the 49ers on Sunday.

Simmons made his impact felt early in the fourth quarter, when he stripped the ball while sacking Brock Purdy, which was recovered by Kaiir Elam. Two plays later, Simmons reported as an eligible receiver and was on the receiving end of a one-yard touchdown pass from Cam Ward. It was the second receiving touchdown of Simmons' seven-year NFL career, with the first taking place during the 2023 season. On the defensive side of things, the 2019 first-rounder is up to 9.0 sacks through 12 regular-season games, which breaks his previous career-best mark from the 2021 campaign (8.5 sacks across 17 regular-season games). Simmons had a disappointing 2023 campaign after signing a four-year, $94 million extension in the offseason. He was limited to 12 games due to injury, though it's unclear if he was ever truly healthy. He had a procedure in April of 2023 to clean up an issue with his ankle, an issue which may have briefly popped up again in late October. One month later, he suffered a knee injury that ultimately derailed his season for good. After a normal offseason, the hope is that Simmons will enter both training camp and the regular season healthy, in which case he should be productive as the centerpiece of the Titans' defensive line.
