2026 Fantasy Outlook
There were some good things to take away from Alcantara's first full season back from Tommy John surgery. He made 31 starts and threw 174.2 innings, which probably exceeded the expectations of even the most optimistic prognosticators. Alcantara's velocity was also essentially all the way back, which, we were reminded by Spencer Strider, isn't always a given. The results, unfortunately, were not there for Alcantara for much of the season. He posted an 8.47 ERA and 1.67 WHIP in the first two months, then was up-and-down for his next 12 starts. Alcantara's final eight outings, though, looked like vintage Sandy, as he held a 2.68 ERA, 0.86 WHIP and 52:10 K:BB over 53.2 frames. The strong finish offers hope that Alcantara can return to peak form consistently in 2026, but it would be risky to draft him as if that's a given. Read Past Outlooks
Control issues in Saturday's loss
Alcantara (2-2) took the loss against Milwaukee on Saturday, allowing three runs on five hits and six walks while striking out one batter across five innings.
Analysis
Alcantara struggled with his command during Saturday's outing, resulting in at least two Brewers batters reaching base safely in four of five innings. He escaped a bases-loaded situation unscathed in the third but gave up his first run in the fourth before yielding a three-run homer to Brice Turang in the fifth frame. Alcantara had as many walks against Milwaukee as he did across his first four starts of the regular season. It was also the first time he did not complete the sixth inning this season, and he now sits with a 3.06 ERA, 1.05 WHIP and 23:12 K:BB across 35.1 innings. Alcantara will look to snap out of his two-game losing streak in his next start, which is slated for next weekend on the road against the Giants.

Alcantara spent the year rehabbing from Tommy John surgery undergone in October 2023. The timing of the procedure limited the right-hander to bullpen sessions, though he began to face live hitters in mid-September. All the progress reports were encouraging, but when he takes the hill in the Grapefruit League, it will be more than16 months since Alcantara threw a ball in anger. Typically, the exercises involved in Tommy John rehab strengthen the area, allowing the pitcher to match, and often add, velocity. The concern is always command and control, which were Alcantara's main assets before the injury. Even with a 98-mph fastball, Alcantara's strikeout rate was above average, but not elite. Workload will also be an issue. Rostering Alcantara is a matter of risk tolerance and level of reliance. It's best to treat him as a luxury item and not a staff fixture.
