2025 Fantasy Outlook
Right elbow inflammation during spring training quickly progressed to Tommy John surgery for Perez, whose 2024 season ended before it began. He'll get a late start to 2025, too, with the Marlins projecting a midseason return for the talented youngster. Perez showed fantasy ace potential at the tender age of 20 in 2023, and if there's a silver lining to him missing the first few months of the 2025 campaign, it's that the kid gloves should be off once he does return. Immediately regaining your stuff and command post-Tommy John surgery is not a slam dunk, of course, and the reality is Perez will be a better fantasy investment in 2026. Inefficient in Friday's start
Perez allowed three runs on two hits and three walks while striking out three over 4.1 innings in a no-decision versus the Diamondbacks on Friday.
Analysis
Perez's pitch count got up early, and he was shaky in the fifth inning, allowing a leadoff homer to Jake McCarthy and a walk before Ronny Henriquez took over in relief. Perez has yet to complete five innings in any of his four starts since returning from Tommy John surgery, but he reached 93 pitches (59 strikes) Friday, his second outing with a roughly full workload. There will still be some rust to shake off, as evidenced by his 6.19 ERA, 1.50 WHIP and 14:10 K:BB over 16 innings. His next start is projected to be at home versus the Twins.

Perez got the call to the big leagues on May 12, less than a month after his 20th birthday. It didn't take long for him to justify the hype; in a six-start stretch from May 28 to June 25, the lanky, 6-foot-8 righty went 4-0 with a 0.27 ERA, 0.88 WHIP and 38:9 K:BB while completing at least five innings in each of those outings. The Marlins monitored his pitch counts closely and built in an extended breather around the All-Star break. Perez would go on to pitch to a 4.26 ERA in eight starts following his return to the majors in August before left hip inflammation ended his season. Home runs were a bit of an issue down the stretch -- he allowed nearly two homers per nine following his return -- but it's understandable that he'd be a bit gassed given his age and the fact that he'd previously topped out at just 77 innings. Between Double-A and the majors, Perez logged 128 innings last season, so a jump to 160-170 frames seems reasonable if far from guaranteed. Sky's the limit long term with his fastball-slider-curveball-changeup mix.
